  2. Signed agreement to acknowledge Saudi Dental Society (Form 2)
  3. CV of Principal Investigator
  4. IRB or other Pertinent Research Approvals or Clearances (as needed)
  5. Research questionnaire (as Google form (with link to share) and word file)
