(Kindly read the following carefully before filling in to complete the form).

1. The principal investigator should be an active SDS member

2. Please provide all the information in English and complete all sections as fully as possible.

3. Please adhere to word limits where they are specified

4. If any important information might be missing, SDS will reject the application form.

5. If any false or misleading information is provided, SDS reserves the right to reject the application.

6. Questionnaire based Studies pertaining to only dental and dental-related research

7. Kindly note, the questionnaire once accepted by SDS for distribution cannot be revised by the research team.
